Home Support Services focus on supporting older adults and those living with disability to remain safe, independent and connected in their own homes and communities.

Are you aged over 65 or living with a long term chronic health condition?

Contact your GP for a referral to NASC (Needs Assessment Service Coordination) and ask for Ngāti Hine Health Trust to be your provider.
